Emergency Medical Refusal for Treatment or Transport

    Evening all:

    In Alamance County, the fire department emt's are simu-dispatched with county EMS for medical calls, 10-50's etc. We function as a first responder, non-transport service. Not wanting to reinvent the wheel, our county fire chiefs are looking for any "Patient Refusal for Treatment or Transport Forms" that you use. Our county EMS uses a modified form from the State OEMS web site. The verbiage on this form does not particularly fit fire service actions. On actual calls, and the patient has some minor injury and refuses, we let the ems unit respond on to the scene and use their form. However, there are calls where it is obvious that there are no injuries and we do not need to tie up a ems transport unit, we then use our fire department form.
